Elevation Hospice of Utah is dedicated to providing the most compassionate, comprehensive and qualified care to people with a life-limiting illnesses, to serve them through their individual journey with dignity, respect and integrity. Elevation was founded over 8 years ago in the State of Utah as a Hospice only agency and has continued with this focus ever since, remaining committed to providing an Elevated care experience for those in our community facing the end of life journey and doing our best to ensure that you have the tools, the education, the knowledge and the support to face the challenges that might arise with confidence, while remaining comfortable and in control of your situation. Our program offers all of the basics that Medicare requires of every hospice agency, such as am Registered Nurse Case Manager, Certified Nursing Assistants to assist you in the home with things such as activities of daily living, hygiene assistance and light housekeeping, Medical Social Support and psychosocial support, Spiritual Counseling and support, Volunteer Services, In Home physician and Nurse Practitioner visits as needed, along with providing any medications that are required to help ensure your comfort and also that are medically necessary for the management of your hospice related diagnosis, as well as any durable medical equipment in the home that will assist you in maintaining your quality of life and functional status at home, items such as hospital beds, wheel chairs, commodes, oxygen, etc.
What differentiates us from the rest is how we utilize our team and how we allocate the tools and the funding that Medicare and other insurances pay for the services we provide. Elevation provides a lower than average Nurse to Patient ratio, allowing our nurses to spend more time with their patients - we ask that each patient is seen at least 2 times per week compared to the average of once per week - of course this is up to you as the patient and also can be more frequently depending on your needs, including up to daily. Our aides usually start off by visiting you around 3 times per week, but they may visit you up to every day of the week as your needs increase in the home to assist with the additional needs that you and your family may have. Our support services team are there to provide psychosocial and spiritual and volunteer support to you, but they are also there to simply listen, be present and provide a time of respite and relief for family members who may need to break away or have someone to talk with, they also can provide licensed counseling services when needed for advanced support. Our MD's and Nurse Practitioners are available for Home Visits and/or tele-health visits when needed as well.
Elevation Hospice also has a variety of programs available to our patients and our community, including our bereavement program, our We Honor Veterans Program and our Final Wishes Program to name a few. We are a charitable organization and are able to assist our community with patients needing end of life hospice care services with or without insurance in most situations. In addition, in response to seeing a huge need in our community, in January of 2021, Elevation launched two new lines of services in the state with Mobile Primary Care and Community based palliative care services, which are now available all along the Wasatch Front for individuals in the community who have a difficult time getting out of their homes and need a primary care provider and/or are dealing with a chronic disease and are still treating and undergoing curative treatment, but could use the additional support of a team of experts to assist them with symptom management and coordination of care throughout the process.
